The Canadian posted the fastest time of 1:31.95 minutes to finish ahead of early leader Hannes Reichelt from Austria who was just 0.02 seconds slower.
The 28-year-old had not topped a World Cup podium since his only previous victory in the downhill in February 2007 at Garmisch-Partenkirchen.
Guay, who finished fifth in both the downhill and Super G at the Vancouver Olympics, is third in the overall Super G rankings with 231 points with Austrian Michael Walchofer leading the way with 300.
There was a tie for third place as Olympic Super G champion Aksel Lund Svindal, who is second overall with 254 points, and Tobias Gruenenfelder both clocked 1:32.26 minutes.
Switzerland's Didier Cuche, who won yesterday's downhill, and Didier Defago, the current Olympic downhill champion, both could not finish the course in Norway.
In the overall standings, leader Benjamin Raich ceded more ground to Carlo Janka of Switzerland after the Austrian could only finish 19th and now has a total of 1019 points, while Janka was seventh and is now only 46 behind.
