Double Olympic medallist Cook, riding Miners Frolic, won her first individual European eventing title in Fontainebleau two months ago and also joined forces with William Fox-Pitt, Oliver Townend and Nicola Wilson to win the team title.
Townend also enjoyed a successful season, winning the HSBC FEI Classics series, in addition to victories at Badminton and Burghley - which set him up for a crack at his sport's Grand Slam in Kentucky next April.
Laura Collett won the BEWA's outstanding under-25 rider award after winning individual and team gold at the European Young Riders' Championship in Belgium.
And Welsh showjumper Carian Scudamore, the European junior champion, claimed the BEWA's under-18 award.
Mistral Hojris was named horse of the year after helping Laura Bechtolsheimer to win bronze and silver at the European Championships in Windsor.
